LLCs:

• LLC Annual List of Members or Managers renewal form
• LLC Annual List of Members or Managers switch form

Limited Partnerships:

• LP Annual List of General Partners renewal form
• LP Annual List of General Partners switch form

Step 2:

Print and complete your form.

Step 3:

Fax completed form to 775-883-4874.

IMPORTANT: We must receive your annual list and payment by 12:00 PM the DAY BEFORE your annual list is due. Failure to fax in your list could result in late fees from the Nevada Secretary of State.
