You can complete the form on-line and print or print it out and complete the form by hand.

Step 3:

Fax completed form to 775-883-4874.

IMPORTANT: We must receive your annual list and payment by 12:00 PM the DAY BEFORE your annual list is due. Failure to fax in your list could result in late fees from the Nevada Secretary of State.
